We must have a patient profile of each person that wants to get their prescriptions filled with us. A Patient Profile consists of a brief medical history, billing information, and authorization. Once we have that on file, we can fill your prescriptions.
Filling Prescriptions - There are no order forms, just mail or fax in a copy of your prescription(s). Suggestion - Once you are on file with us, you can have your doctor fax us your prescription.
